Chosen Name

    Complete and submit this form to the Registrar's Office. Please note that this form is not for students submitting a legal name change. Students submitting a legal name change should use the Legal Name Change form.

    Important Information Regarding Chosen Name

    This form is to be used by students who have not legally changed their names but who wish to use a chosen name across campus platforms. Chosen names may also be used in mailings and other communications to students and student's families. The campus platforms that will be updated with the new information include:

    - Class Rosters
    - Email display names
    - Net ID
    - Student ID cards
    - Housing Rosters
    - Library Records
    - Peer Mentor Group Lists
    - JobX
    - Athletic Rosters
    - Canvas (For this platform specifically, students can always update their name and pronouns within their Canvas profile directly).
